Inclusive Aesthetics Memberships

For a $75 monthly fee, members receive consistent laser maintenance and significant savings on all other advanced device treatments:

• Monthly Session: Includes one Laser Hair Removal session every 3–4 weeks for one "Small Area" (up to a $100 value).
• Exclusive Discount: 25% OFF all laser and device treatments all year long.
• Eligible Small Areas: Lip, chin, sideburns, ears, brows, neck line, areola, "happy trail," fingers/hands, or toes/feet.

Our Injection Membership is designed to give you the best value on your favorite treatments year-round for a $250 annual fee. Benefits include:

• Birthday Perk: One complimentary Laser Genesis or Skinwave treatment during your birthday month.
• Exclusive Filler Savings: $100 off per syringe of Dermal Filler, $150 off Voluma & Volux, and $100 off two syringes of SKINVIVE™.
• No Injection Fees: We waive injection fees for all pre-paid filler appointments.
• Member-Only Neurotoxin Pricing:
  • Botox: $12/unit
  • Jeuveau: $11/unit
  • Dysport: $4.50/unit

Your membership fee of $50 is charged automatically on the 1st day of each month.

• Auto-Renewal: Your membership renews monthly until a termination request is submitted and processed.
• Payment Authorization: By enrolling, you authorize Inclusive Aesthetics LLC to charge your monthly fees via Electronic Funds Transfer (EFT) using your provided payment information.
• Account Management: You may update your payment method at any time through your Aesthetic Record portal or by contacting Inclusive Aesthetics reception.

Your membership fee of $75 is charged automatically by the 1st day of each month.

• Auto-Renewal: Your membership will continue to renew monthly until a formal termination request is submitted.
• Payment Authorization: By enrolling, you authorize Inclusive Aesthetics LLC to charge your monthly fees via Electronic Funds Transfer (EFT) using the payment information on file.
• Managing Your Account: You can update your credit or debit card information at any time through your Aesthetic Record portal or by contacting our reception staff.

The membership fee is $250 per year. By signing up, you authorize Inclusive Aesthetics LLC to charge this annual fee to your provided payment method via Electronic Funds Transfer (EFT).

• Auto-Renewal: Your membership will automatically renew each year until a written termination request is received.
• Managing Payments: You are in full control of your billing. Changes to your credit or debit card information can be made through your Aesthetic Record portal or by contacting our reception staff.

All Inclusive Aesthetics memberships (Injection, Laser, and Facial) are subject to the following terms:

• Non-Transferable: Memberships and their associated benefits are exclusive to the account holder. They are non-transferable and may not be shared with other individuals.
• Non-Refundable: All membership payments are final and non-refundable once processed.
• Third-Party Purchases: Memberships may be purchased as a gift or on behalf of another person, provided written consent is given by the individual receiving the membership.
• Policy Updates: Inclusive Aesthetics LLC reserves the right to modify clinic policies, regulations, and pricing at any time. Members will be provided with reasonable notice regarding any significant changes.

Your membership will automatically be renewed at the end of each month, unless canceled via Aesthetic Record through your patient profile or a cancelation request is submitted to Inclusive Aesthetics at info@inclusiveaestheticsspa.com.

To cancel any membership (Injection, Laser, or Facial), we require 30 days' notice prior to your next scheduled payment. Please note the following:

• Refunds: No refunds will be issued once a membership fee (monthly or yearly) has been charged or paid in full.
• Billing: Inclusive Aesthetics collects membership dues on your scheduled billing date.
• Processing: Once written notice is received, we will implement your cancellation request immediately, though the 30-day notice period still applies to any payments scheduled within that window.

We understand that circumstances may arise where you need to take a short break. Facial and Laser memberships may be paused under the following guidelines:

• Frequency & Duration: Memberships can be paused once per calendar year for a minimum of 1 month and a maximum of 6 months.
• Benefits: During the pause period, membership services and benefits are temporarily suspended and cannot be used.
• Billing: At the conclusion of your pause term, monthly dues will automatically resume on your next scheduled billing date via electronic funds transfer.
• Fees: There are no setup or cancellation fees associated with pausing your account.

Please Note: The Injection Membership is a yearly plan and is not eligible for pausing.

Please read this agreement carefully before signing. By accepting this membership, you acknowledge that you have been fully informed of all Inclusive Aesthetics LLC terms, conditions, benefits, and limitations.

• Binding Agreement: This document is a legally binding agreement upon its acceptance by Inclusive Aesthetics LLC. You are entitled to a completed copy of this agreement, which is available digitally within your Aesthetic Record patient profile.
• Acknowledgment of Terms: The undersigned member acknowledges receipt of all membership terms and conditions and agrees to be bound by them in their entirety.
• Minor Policy: Any member under the age of 18 must have a parent or legal guardian co-sign this agreement. The parent or guardian must be present during all office visits and, along with the member, agrees to be bound by all terms and conditions of this agreement.
